Do I need every visa page translated, or just the bio-data page?

Only the bio-data page by default; endorsement pages only if they contain Thai-language stamps. Visa stickers from other countries are usually in English or the local language and do not need re-translation.

My name in the passport is spelled differently to my Kor Ror 3 — what do I do?

This is the passport name of record; the Kor Ror 3 is the birth-name of record. We add a spelling-reconciliation note in both translations so the two documents cross-reference each other.

Can I use a translation of my expired passport?

Yes for historical identity evidence (e.g., proving continuity of identity across a prior visa lodgement). Not for current-day identity — DHA wants the passport currently in your possession.
